Preschool & Kindergarten Screenings

Pre-entry screenings are standard practice for any child entering preschool or kindergarten at a public school. Screenings are conducted by the Parents as Teachers (PAT) team. No preparation is needed for your child to attend the screening and is performed at no cost. The PAT educators will simply evaluate things like hearing, vision and developmental milestones through play activities.

Preschool Screening Dates

April preschool screening appointments are all booked up, but don't worry we've opened up one date in May. It's first come, first serve.

Eligibility and What to Bring

There is no preparation needed for your child's screening, but your child must meet the age requirements and please bring the following items:

 

Eligibility requirements:

  • Preschool – child must be four years old before August 1, 2024
  • Kindergarten – child must be five years old before August 1, 2024

Please bring:

  • Proof of Residency (Rental Contract, Real Estate Contract signed by all parties, Utility Bill, Deposit Receipts, Bank Statement, Payroll Check, Driver’s License, or employment documents)
  • Birth Certificate (Birth Certificate, Hospital Document, Passport or Affidavit)
  • Immunization Records
  • Social Security Card (optional)
  • Medicaid Card (if applicable)
  • Custody Papers (if applicable)
